Bonum Health Terms and Conditions

Bonum Health is not an emergency care service.
IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY

Subscriptions:

Each subscription comes with three (3) virtual consults (“visits”) with a provider from our third-party provider network, per month. Should a user exceed three (3) visits in a month, their subsequent visits will be billed at the per-consult rate. Subscriptions are billed monthly and can be cancelled at any time through the web portal or by emailing notice of request to cancel to support@bonumhealth.com. Lab testing and/or prescription medication is not covered under your subscription.

Bonum Health requires one account per person utilizing its services. Upon registering with Bonum Health you agree that all financial transactions, subscription or per-consult, will be automatically and electronically debited to whichever card or account entered when registered. By agreeing to these Terms and Conditions, you affirm that you will not use your account for another person, you will not represent someone else’s symptoms as your own, and you will not share your profile with anyone else. You agree that violating these, or any other terms, can result in the immediate freezing, suspending, or deactivation of your account without notice.

Bonum Health reserves the right to limit, suspend, or cancel a user’s account upon suspicion that services are being abused or utilized conversely to what is intended. Any account suspension or cancellation will be communicated via email to the user along with an outline of the reason for the account status change. The physicians are members of a third-party company contracted by Bonum Health. Bonum Health is not a physician provider service, and Bonum Health assumes no liability for diagnoses, prescriptions, advice, or any exchange of information between the patient and provider. By agreeing to these Terms and Conditions you agree that Bonum Health holds no liability for information, advice, or services set forth by the provider during and following a visit.

By agreeing to these Terms and Conditions, you acknowledge that visits are not meant to replace primary care provider visits and follow up consultations with your primary care provider may be recommended; nor is it intended to treat emergency services. You are also acknowledging that Bonum Health is a technology company connecting patients to a third-party network of providers and all liability relating to medical advice, diagnoses, or prescriptions is solely on the physician providing the consultation and Bonum Health is not liable for any adverse outcomes related to said visit.

Per-Consult:

For patients who do not want a subscription at this time, Bonum Health offers a per-consult pricing model. The price of each consult varies based on specialty, and prices are subject to change without notice.

By agreeing to these Terms and Conditions, you affirm that you will not impersonate another person, portray someone else’s symptoms as your own, or attempt to have more than one patient consulted at a time. Telemedicine Consultations:

Telemedicine consultations are conducted by remote, US board-certified physicians. These physicians are members of a third-party company contracted by Bonum Health. Bonum Health is not a physician provider service, and Bonum Health assumes no liability for diagnoses, prescriptions, advice, or any exchange of information between the patient and provider. By agreeing to these Terms and Conditions, you agree that Bonum Health holds no liability for information, advice, or services set forth by the provider during and following a telemedicine consult.

Your image and healthcare information will be transmitted electronically through videoconference to the telemedicine consultant for assessment, diagnosis, and treatment recommendations. By agreeing to these Terms and Conditions, you acknowledge that telemedicine consultations are not meant to replace the use of a primary care physician and that video and/or audio consultations come with inherent limitations, including but not limited to, inability to use touch or smell to assist in diagnosis, using physical manipulation assessment techniques, or any in-person means for collecting perceptive data from the patient.

By agreeing to these Terms and Conditions, you understand that there are potential risks and limitations to the telemedicine technology, including interruptions, unauthorized access, and technical difficulties. Bonum Health will do everything within its reasonable knowledge and control to maintain all security and compliance certifications. By agreeing to these Terms and Conditions, you acknowledge the inherent risks and limitations that exist while utilizing any online platform. Any and all concerns about data security can be relayed 24/7/365 to support@bonumhealth.com. Agreeing to these Terms and Conditions gives Bonum Health permission to securely collect any information deemed necessary for operation of its health services, including but not limited to, providing protected health information (“PHI”) to the provider network and their support staff for scheduling and health records review. Bonum Health will also collect non-PHI data (including but not limited to full name, mailing address, email address, and phone number) for the operation of the site, marketing and affiliate programs.
